Florida Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and collaborate on ongoing care plans, ensuring adequate access to services
• Provide education to members and families on healthcare processes and available resources

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• OB, NICU, or Pediatric experience highly preferred
• Must reside in the State of Florida
