Job Summary

A company is looking for a Family Nurse Practitioner for virtual wellness visits through December 2025.

Key Responsibilities

• Conduct virtual wellness visits, health assessments, and proactive patient care
• Collaborate with clinical in-home team members to evaluate patient needs and coordinate care
• Provide comprehensive care management, including medication management and referrals to specialists

Required Qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License
• Exhibit high levels of professionalism, integrity, and compassion
• Comfortable using technology for charting and communication
• Ability to work independently or collaboratively as needed
