Job Summary

A company is looking for a Telehealth Provider (MD) to conduct remote healthcare visits and manage care plans for patients.

Key Responsibilities

• Conduct Telehealth E/M visits with Medicare beneficiaries
• Create comprehensive care plans for Chronic Care Management (CCM)
• Collaborate with the care team to ensure appropriate patient referrals and care

Required Qualifications, Training, and Education

• Medical licensure/certification in Georgia or Missouri, or willingness to obtain
• Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Experience in telehealth, wellness visits, and preventative care
• Compliance with CEUs and licensure requirements in all states where licensed
• Comfortable using electronic health record (EHR) systems and medical software
