Health Assessor

Job Summary

A company is looking for a Member Health Assessor.

Key Responsibilities

• Conduct health risk assessments and gather information from members and stakeholders
• Coordinate logistics to support care plan goals and assist with health education and wellness materials
• Maintain documentation and contribute to the development of care plans based on assessment outcomes

Required Qualifications

• High School Diploma or General Education Diploma (GED) is required
• Minimum of two (2) years of experience in community settings with at-risk populations is preferred
• Proficient in Microsoft Office, including Outlook, Word, and Excel
• Sensitivity to and experience working within different cultures
• Ability to work independently and as part of a team
