Job Summary

A company is looking for an RN Plan of Care Coordinator for remote weekend shifts.

Key Responsibilities

• Oversee the quality of home health patient care plans in collaboration with clinicians
• Edit and build detailed, individualized plans of care using referral documents and assessments
• Maintain case status information and notify leadership of problematic trends during case reviews

Required Qualifications

• Associate degree in nursing with current unencumbered licensure in the state of residence; Bachelor's degree preferred
• Minimum of 1 year of healthcare experience required; home health experience preferred
• Backoffice experience is necessary for this role
