Job Summary

A company is looking for a Clinical Pharmacist - Remote.

Key Responsibilities:

• Conduct telephonic consultations with members for comprehensive medication reviews and adherence consults
• Document care delivered and communicate essential information to primary care providers
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens

Required Qualifications:

• Must be 18 years of age or older
• Current and unrestricted Pharmacist license in the state of residence
• 1+ years of experience working remotely/independently in a cross-functional team environment
• Solid knowledge of disease state management guidelines and MTM services
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
