Job Summary

A company is looking for a Social Worker Casual.

Key Responsibilities

• Provide developmentally appropriate care and plan for safe discharge and continuity of care for diverse patient populations
• Advocate and educate patients and families on self-care management, health management, and community resources
• Facilitate patient care conferences and manage transitions of care, including psychosocial support and resource coordination

Required Qualifications

• Bachelor's or Master's Degree from a school of social work accredited by the Council of Social Work Education
• Social Work license in the State of NE and/or IA
• Experience in care coordination and discharge planning
• Knowledge of community resources related to clinical social work
• Understanding of federal/state/local regulatory agency guidelines and accreditation requirements
