Job Summary

A company is looking for a Social Worker II - LCSW Behavioral Health - Remote Contract.

Key Responsibilities

• Provide culturally competent direct service to patients and their families using various social work modalities
• Utilize high risk criteria to identify patients needing social work assessment and intervention, advocating for their access to health care
• Formulate and implement treatment and discharge plans while maintaining professional patient care documentation

Required Qualifications

• Master of Social Work required
• Current License to practice as a Clinical Social Worker in New York State required
• 1-3 years of relevant experience required
• Specialized certifications as needed
