New York Licensed Health Home Manager

Job Summary

A company is looking for a Health Home Care Manager (Spanish-speaking).

Key Responsibilities

• Complete initial and ongoing assessments to guide individualized care planning
• Coordinate services across medical, behavioral health, substance use, and social support systems
• Help clients navigate healthcare, secure housing, schedule appointments, and access benefits

Qualifications

• Bachelor's degree in health or human services plus 2 years of relevant experience, or a Master's degree in health or human services plus 1 year of relevant experience, or a Licensed Registered Nurse (RN) plus 2 years of relevant experience
• Experience with Medicaid populations, chronic conditions, and social determinants of health
• Proficiency in Microsoft Office and EHR systems
• Ability to travel across NYC for community and home visits
• Bilingual in Spanish required
