Job Summary

A company is looking for a Senior Care Manager (RN) - Transition of Care.

Key Responsibilities

• Develops and assesses ongoing care plans and collaborates with providers to address members' needs
• Coordinates care between members, families, and care providers to ensure person-centered services
• Monitors member status and revises care plans as necessary to meet goals and needs

Required Qualifications, Training, and Education

• Active New York State Registered Nurse License
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• Experience in care management and collaboration with healthcare providers
• Knowledge of state and federal regulations related to healthcare services
