Job Summary

A company is looking for a Care Manager Bilingual in English and Korean - 100% Remote.

Key Responsibilities

• Advocates and educates beneficiaries on services and self-management techniques
• Conducts assessments and develops care plans in alignment with treatment goals
• Coordinates care among providers and documents all interactions in the appropriate systems

Required Qualifications

• NYS RN or LCSW or LMSW (any state)
• Fluency in Korean
• For Behavioral Health positions: 3 years of work experience in a mental/behavioral health or addictions setting
• 1 year of pediatric clinical field experience for PEDS positions
• Experience managing member information in a shared network environment
