North Carolina Licensed Case Manager

Job Summary

A company is looking for a Remote Case Manager.

Key Responsibilities:

• Collaborate with a multidisciplinary healthcare team to monitor utilization patterns and facilitate appropriate care delivery for selected members
• Conduct comprehensive assessments of members' health status, including clinical history and support needs
• Coordinate with healthcare providers to develop and implement treatment and discharge plans

Required Qualifications:

• Active RN license (North Carolina or compact state required)
• 3+ years of inpatient care experience, including critical care
• Prior telephonic case management experience is a plus
• Must reside in the Eastern Time Zone
