Job Summary

A company is looking for a Nurse Practitioner - PRN Telehealth.

Key Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, and documentation
• Collaborate with physicians, case managers, and caregivers to coordinate patient care
• Provide services through home visits, skilled facility visits, and telehealth visits

Required Qualifications

• Master's Degree or equivalent experience with satisfactory completion of NP licensure
• Current RN and NP licensure in state of practice with prescription authority
• Board certified by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum of 1 year experience as a Nurse Practitioner
