Job Summary

A company is looking for a Medicare Care Coordinator Associate.

Key Responsibilities

• Provides a single point of contact for all questions or inquiries
• Conducts surveys and assessments with Enrollees and/or their caregivers
• Reviews and updates Individualized Care Plans periodically

Required Qualifications, Training, and Education

• Bachelor's degree or higher in a health or social services-related field, or an associate prepared RN or LPN with a bachelor's degree in another field
• 3 to 5 years of case management experience preferred
• 1 to 3 years of experience in social services and/or care management/coordination in a home/community setting
• Medical Assistant, Home Health Aide, or Nursing Assistant experience is strongly preferred
• Experience in a remote/telephonic environment preferred
