Performance Enablement Specialist

Job Summary

A company is looking for a Performance & Enablement Specialist.

Key Responsibilities

• Visit provider practice offices to advise on population health strategies and practice transformation
• Assist providers and staff in understanding value-based performance initiatives to enhance patient care
• Collaborate with primary care providers to identify process improvements and enhance patient outcomes

Required Qualifications

• Bachelor's Degree in healthcare business or related field; or 2 years of related experience
• Experience working with physicians or in healthcare settings
• Advanced certification or formal training in Population Health, Managed Care, or Value-based Care (preferred)
• Change Management Training (preferred)
• Lean 6 Sigma Green Belt Certification (preferred)
