Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develops and facilitates complex care management activities for members with physical needs
• Coordinates care plans and collaborates with providers to address member's unmet needs
• Monitors member status and revises care plans as necessary to meet goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
