Registered Nurse Intensive Care Manager

Job Summary

A company is looking for an Intensive Care Manager RN in Population Health.

Key Responsibilities:

• Coordinate and monitor the health needs of patients, conducting comprehensive assessments to identify gaps in care
• Develop and implement care plans to maximize healthcare outcomes and facilitate wellness
• Collaborate with healthcare providers and manage patient outreach and referrals to community resources

Required Qualifications:

• Associates Degree required; Bachelor's Degree preferred
• 3 years of relevant experience required; experience in care management or clinical nursing specialty preferred
• Licensed Registered Nurse or Multi-State License required upon hire
• Basic Life Support certification required upon hire
