• Provide telephonic education and support to members with chronic illnesses
• Conduct assessments and develop personalized health education plans for Medicare members
• Collaborate with healthcare professionals to ensure members meet their treatment goals and access necessary resources

Required Qualifications

• 3+ years of related work experience in health education and chronic population management
• Related Bachelor's degree in Health Education or a health-related field
• Respiratory Therapist Certification
• Experience working in a health plan setting or with telephonic health education preferred
• CDCES Certification is preferred
