Key Responsibilities

• Engage and educate caregivers via phone and digital technology to improve their wellbeing
• Manage a caseload and assess caregiver and care recipient needs, assisting in care plan goals
• Document coaching activities and collaborate with stakeholders to identify problems and solutions

Required Qualifications and Education

• 4+ years of experience in care planning or case management
• Experience working with the elderly or adults with disabilities
• Knowledge of industry concepts and principles related to coaching support
• Proven experience in accessing community resources
• Ability to navigate and teach the use of mobile applications
