Required Qualifications and Education
- 4+ years of experience in care planning or case management
- Experience working with the elderly or adults with disabilities
- Knowledge of industry concepts and principles related to coaching support
- Proven experience in accessing community resources
- Ability to navigate and teach the use of mobile applications
