Care Coordinator

Job Summary

A company is looking for a Care Coordinator to join their Care Management team.

Key Responsibilities

• Engage with high-risk patients to ensure follow-up appointments and close care gaps
• Collaborate with care team members and external vendors to support patient care
• Request medical records to ensure continuity of care and assist nursing and social work teams

Required Qualifications

• 3+ years of experience in a medical office or healthcare setting
• 2+ years working as a Licensed Practical Nurse (LPN), Medical Assistant (MA), or Community Health Worker (CHW) preferred
• Comfort with technology, including Microsoft Office products
• Prior experience using electronic health records for data capture and reporting
