A company is looking for a Care Management Support Coordinator II to assist with administrative care management activities.
Key Responsibilities
- Perform outreach to members via phone to support care plan next steps and address concerns
- Connect members to health plan and community resources to enhance customer care
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Bilingual (Russian) highly preferred
