Care Navigator

Job Summary

A company is looking for a Care Navigator who will assist patients with complex musculoskeletal conditions in accessing healthcare and community services.

Key Responsibilities:

• Perform outbound calls to patients to review care plans and connect them with resources
• Verify insurance eligibility, provide cost estimates, and obtain prior authorizations
• Facilitate referrals and scheduling with external providers, and document all patient interactions in the EMR

Required Qualifications:

• High school graduate or equivalent
• Medical Assistant degree or similar healthcare non-licensed degree
• 2+ years of experience as a Medical Assistant or Referral Coordinator
• Experience with insurance eligibility and benefit verification
• Clinical knowledge of musculoskeletal conditions and related healthcare services
