Certified Medical Assistant Care Coordinator

Job Summary

A company is looking for a Remote Care Coordinator (Certified Medical Assistant).

Key Responsibilities

• Conduct monthly phone calls with patients and healthcare providers to ensure patient needs are met
• Create and revise personalized care plans in coordination with healthcare practices
• Identify social determinants of health and collaborate with community resources to address patient needs

Required Qualifications

• At least 1 year of experience as a CMA/RMA, LPN, or RN with an active accreditation/professional license
• Experience with multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite
• Highly self-motivated and organized, with the ability to work independently
• Experience serving a geriatric population is a plus
• Case management or care coordination experience is a plus
