Job Summary

A company is looking for a Clinical Registered Nurse - Remote.

Key Responsibilities

• Provide high-quality telephonic support and coordinate with healthcare team members for total healthcare management
• Identify participants at risk for increasing acuity and intervene according to disease management program guidelines
• Develop and facilitate care plans addressing participants' healthcare needs and barriers to accessing services

Required Qualifications

• Current RN license appropriate for the state of employment
• High School diploma and a Diploma, Associate's, or Bachelor's Degree in Nursing (BSN preferred)
• Minimum of 3 years recent clinical experience
• Basic knowledge of physiology and chronic diseases
• Familiarity with basic computer skills and Windows operating system
