Job Summary

A company is looking for a Triage Support RN, responsible for providing telephone triage and care to Medicare patients with chronic conditions.

Key Responsibilities

• Provide non-face-to-face telephone triage and care planning for Medicare patients with multiple chronic conditions
• Collaborate with healthcare teams and document patient encounters in charts
• Educate patients and manage urgent care queues to coordinate services and follow up on medical issues

Required Qualifications

• Current COMPACT RN license
• Willingness to obtain multi-state licenses
• Knowledge of chronic care conditions and relevant medications
• Ability to follow clinical triage protocols and document accurately
• Willingness to work an on-call rotating schedule
