Job Summary
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Perform outreach to members and support care plan next steps and resource connections
- Serve as the front-line support for member and provider inquiries, explaining care plan procedures
- Document and maintain non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
- High School diploma or GED required
- 1 - 2 years of related experience preferred
- Call center experience with a high volume of calls is necessary
- 2-3 years of experience in healthcare medical office, facilities, or administration duties strongly preferred
- Knowledge of health plan activities and community resources is beneficial
