Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical health needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and collaborate on ongoing care/service plans with providers and community resources
• Monitor member progress and adjust care plans as necessary to meet goals

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Experience in OB and Physical Health (ICU, ER, Med-Surg, Ortho, etc.) preferred
