Health Coach (NBHWC Certified)

Job Summary

A company is looking for a Health Coach to support patients in managing their health through education and personalized action plans.

Key Responsibilities

• Serve on an interdisciplinary care team to provide patient health coaching
• Empower patients to set and achieve their health and wellness goals
• Document services provided in accordance with program and compliance requirements

Required Qualifications

• Bachelor's degree in healthcare or related field preferred
• Minimum of 3 years' experience in chronic condition clinical coaching or relevant healthcare coaching experience
• National Board for Health and Wellness Coaching (NBHWC) Certified or willing to obtain certification within 3 months of employment
• Deep understanding of positive psychology, behavior change, and motivation
• Comfortable with digital technology and able to troubleshoot technology issues
