Job Summary

A company is looking for a Community Health Worker for a 2-month remote contract position.

Key Responsibilities

• Complete access to care assessments over the phone with KY Medicaid members
• Enter assessment responses into Community Care to address care gaps
• Collaborate with the Quality & Enrollee Team to support members effectively

Required Qualifications

• Healthcare background preferred
• Experience as a Community Health Worker (CHW) is preferred
• Familiarity with KY Medicaid is preferred
• Experience addressing Social Determinants of Health (SDOH) needs is preferred
• Proficiency with technology and organized documentation skills
