Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans and collaborate with healthcare teams to support member transitions
• Facilitate education and resources to promote wellness and improve quality of care

Required Qualifications

• Master's degree in Behavioral Health, Social Work, or an accredited Nursing degree
• 2-4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
• Ability to work weekends and within the Central Time Zone (CST)
