Licensed Concierge Care Manager

Job Summary

A company is looking for a Concierge Care Manager to provide care management services for members with complex healthcare needs.

Key Responsibilities

• Administer high-touch case management and coordinate treatment plans for high-risk patient populations
• Collaborate with external providers and community services to facilitate care delivery
• Engage in team training and prepare for quality assurance audits

Required Qualifications, Training, and Education

• Associate's degree in nursing or a graduate of an accredited school of nursing; Master's degree in Social Work, Psychology, or Counseling for specific divisions
• 5 years of case management experience as an RN or 2 years in a health insurance environment
• Active, unrestricted RN licensure in the state of hire or equivalent licensure in social work, psychology, or counseling
• Nationally recognized Case Management certification to be obtained within 2 years of hire
• Working knowledge of Microsoft Office and familiarity with quality improvement processes
