Job Summary

A company is looking for a Care Coordination Nurse I- AWV (LPN).

Key Responsibilities

• Manage inbound and outbound patient interactions to complete Medicare Annual Wellness Visit forms
• Conduct phone interviews to gather patient medical history and assist with appointment education
• Ensure compliance with privacy standards and complete documentation as required

Required Qualifications

• Minimum 3 years of experience in a doctor's office or hospital setting
• Minimum 3 years of experience working with patients
• High school diploma required; college or some college preferred
• Licensed Practical Nurse (LPN) license required
• Typing speed of at least 55 wpm
