Job Summary

A company is looking for a Care Navigator- LPN/LVN.

Key Responsibilities

• Manage physiological markers such as blood pressure, weight, and blood glucose with clinical appropriateness
• Perform monthly wellness calls and lead collaborative discussions to define health goals
• Provide preventive health education and manage patient messaging and alerts

Required Qualifications, Training, and Education

• Unrestricted LPN/LVN license in a compact state and Michigan
• 1-3 years of clinical experience in patient or disease management
• Experience with virtual visits and telephonic care management
• Ability to work independently in the EST time zone between 8:00 AM - 5:00 PM
• Spanish speaking is a plus
