A company is looking for a Complete Weight Management Navigator, LPN with NJ license.
Key Responsibilities
- Serve as a point of contact for patients and referring physicians, facilitating access to clinical and supportive care services
- Coordinate patient appointments and follow patients throughout the care continuum, collaborating with healthcare team members
- Develop and present patient education programs and maintain positive relationships with key customers in the healthcare setting
Required Qualifications, Training, and Education
- A minimum of 2-3 years of customer service or call center experience as an LPN
- Graduate of an accredited LPN program
- Must possess a valid NJ LPN license
- Demonstrates knowledge of nursing skills, hospital practices, and standards
- Ability to use general office equipment, including a multi-line telephone system
