A company is looking for a Telephonic Case Manager - Remote.
Key Responsibilities
- Make outbound and receive inbound calls to assess members' health status
- Identify gaps or barriers in treatment plans and provide patient education for self-management
- Support members with condition education, medication reviews, and connections to resources
Required Qualifications
- Current, unrestricted RN license in the state of New York
- 3+ years of experience in case management, hospital, acute care, or direct care settings
- Ability to type and navigate a Windows-based environment
