Oklahoma Care Management Associate

Job Summary

A company is looking for a Care Management Associate, Engagement Hub.

Key Responsibilities

• Conduct telephonic outreach to engage and enroll eligible members in care management
• Facilitate case assignments and connect members to care managers through warm transfers
• Maintain accurate documentation of member interactions to ensure compliance with regulatory standards

Required Qualifications

• MUST LIVE IN OKLAHOMA
• 2 - 4 years' experience in the healthcare field is preferred
• High School diploma, G.E.D. or equivalent is required
• Experience with Microsoft Word, Outlook, and Excel is preferred
• Ability to work independently and meet deliverables in a dynamic environment
