Job Summary

A company is looking for a Telephonic Outreach Specialist to engage with members eligible for services and assist them throughout the intake process.

Key Responsibilities

• Provides telephonic/virtual outreach and engagement services for eligible members
• Initiates the intake process and communicates urgent needs to care teams
• Collects data and organizes information within the Commons system

Required Qualifications

• 1 year of experience in a similar role
• Ability to commute to specified regions in Maryland or Virginia
• Experience with data collection and management systems
• Familiarity with healthcare or community service environments
• Ability to work collaboratively with care teams
