Job Summary
A company is looking for a Part-Time Case Management Coordinator.
Key Responsibilities
- Manage a caseload of 20-25 cases focusing on care coordination or continuity-of-care
- Utilize various sources of information to implement case management care plans as directed by a supervisor
- Maintain detailed documentation of care plan progress and submit finalized reports every 4 weeks
Required Qualifications, Training, and Education
- High school diploma or GED
- 2-3 years of experience in customer service, claims, and/or triage
- Coordination of care experience in an insurance or hospital setting
- Computer proficiency with Microsoft Office Suite and Outlook
- Knowledge of medical claims and coding (ICD-10, CPT, HCPCS) preferred
