Key Responsibilities

• Provide health coaching and case management services to members across various health stages
• Develop and monitor care plans while facilitating communication between patients, families, and healthcare teams
• Identify barriers to adherence and assist members in accessing resources for achieving personal health goals

Required Qualifications

• Current, active, and unrestricted Pennsylvania (PA) Registered Nurse licensure
• Three years of experience in clinical, case management, and/or disease management
• Ability to work flexible hours
• Basic computer skills
• Strong clinical knowledge
