RN Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develop and facilitate complex care management activities for members with physical needs
• Collaborate with providers to create and update personalized care plans based on member needs
• Provide ongoing follow-up and resource support to ensure adequate care and services for members

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
