A company is looking for a Coordinator - Social Services.
Key Responsibilities
- Obtain and share information on benefits and community resources for patients post-discharge
- Assist in processing Medicaid applications and other necessary documentation for patient benefits
- Collaborate with nurse coaches to provide patients with information on community resources for recovery support
Required Qualifications, Training, and Education
- Degree in social work or a related field
- Deep knowledge of community amenities and resources
- Strong understanding of medical terminology and planning
- Experience in recognizing Health Plan benefits and community resource needs
- 5+ years of experience in community resource management or Health Plan benefit management
