State Licensed Care Team Lead

Job Summary

A company is looking for a Care Team Lead (MST or PST).

Key Responsibilities

• Deliver full-scope CCM services to a partial patient load while managing a team of care managers
• Support interviewing, hiring, and training efforts for new care managers to ensure their success
• Oversee staff education and adherence to regulatory requirements while advocating for team effectiveness

Required Qualifications

• Prior experience in a CCM and/or RPM program
• At least 6 months experience in a remote or telehealth position
• Minimum of one year in a people management role
• Active healthcare license or certificate (CNA, CMA, RMA, LPN, LVN, RN, LCSW, QMHP)
• Ability to work standard business hours in MST or PST time zones
