State Licensed Population Health Coordinator

Job Summary

A company is looking for a Population Health Coordinator to support high-quality care delivery to specific member populations.

Key Responsibilities

• Manage data and registries to track member progress and care quality
• Coordinate care for complex members and assist in training staff on care models
• Provide direct member care through outreach, screenings, and educational support

Required Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) for Medical Focus; licensed clinical social worker for Mental Health Focus; or certified peer/community health worker for Social Care Focus
• At least 5 years of experience in relevant fields such as addiction medicine, mental health, or case management
• Bachelor's degree in Nursing preferred for Medical Focus
• Proficiency with EMRs and Microsoft Office/Google Suite
• Access to reliable internet and telephone services for remote roles
