Job Summary

A company is looking for a Telecare Care Coach - Temporary.

Key Responsibilities
  • Manage patient demographics, schedule appointments, and maintain accurate patient information
  • Coordinate meetings and documentation for the Interdisciplinary Care Team
  • Respond to inquiries and perform outreach to patients and their families
Required Qualifications
  • High School diploma required
  • Three (3) years of customer service or transaction processing experience in a Managed Care setting
  • Knowledge of Medicare Advantage and Medicare Advantage Part D health plans preferred
  • Proficient in MS Office applications including Word, Excel, and Access
  • Ability to work independently and manage a high workload in a fast-paced environment
