A company is looking for a Telecare Care Coach - Temporary.
Key Responsibilities
- Manage patient demographics, schedule appointments, and maintain accurate patient information
- Coordinate meetings and documentation for the Interdisciplinary Care Team
- Respond to inquiries and perform outreach to patients and their families
Required Qualifications
- High School diploma required
- Three (3) years of customer service or transaction processing experience in a Managed Care setting
- Knowledge of Medicare Advantage and Medicare Advantage Part D health plans preferred
- Proficient in MS Office applications including Word, Excel, and Access
- Ability to work independently and manage a high workload in a fast-paced environment
