RN Post-Acute Care Manager

Job Summary

A company is looking for a Post-Acute Care Manager - Population Health.

Key Responsibilities

• Run daily reports for transitions of care and review goals of care
• Communicate regularly with skilled nursing facilities to encourage progression of care
• Conduct comprehensive assessments and coordinate referrals and transitions of care

Required Qualifications, Training, and Education

• Current RN License in the state of practice or covered by compact
• Bachelor of Science in Nursing (BSN) preferred
• 2 years of nursing experience required
• 1 year of experience in skilled nursing settings preferred
• EPIC Electronic Health Record training preferred
