Illinois Licensed Palliative Care Nurse Practitioner

Job Summary

A company is looking for a Palliative Care Nurse Practitioner (IL-Licensed).

Key Responsibilities

• Conduct specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team
• Oversee interdisciplinary case conferences to create and monitor individualized care plans for members with advanced serious illness
• Provide education on palliative care to Community Health Workers and Oncology Nurses, focusing on cancer-related symptoms and advance care planning

Required Qualifications, Training, and Education

• At least 3 years of nurse practitioner experience, with a minimum of 2 years in palliative care
• Advanced certified hospice and palliative nurse practitioner (ACHPN) certification
• Unrestricted nurse practitioner license with a willingness to obtain additional state licenses as needed
• Experience in an outpatient oncology setting is preferred
• Familiarity with technology and virtual care platforms
