State Licensed MTM Pharmacist

Job Summary

A company is looking for an MTM Pharmacist - Independent Contractor.

Key Responsibilities

• Conduct patient outreach through phone calls to provide medication therapy management services
• Offer expert support by performing comprehensive and targeted medication reviews, medication reconciliation, and counseling on medical device usage
• Document clinical recommendations and patient care notes in a clear and professional manner

Required Qualifications

• Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy (BS Pharm) degree
• Active pharmacist license to practice in at least one US state
• Ability to obtain licensure in additional states as needed
• Strong telephonic skills for servicing an elderly population
• Experience in conducting Medication Therapy Management services is preferred
