Job Summary

A company is looking for a Spanish & English speaking LPN in Tennessee.

Key Responsibilities:

• Serve as the primary point of contact for patients enrolled in RPM and CCM programs
• Review transmitted patient health data daily, identify red flags, and escalate issues to the clinical team
• Provide culturally competent health education in Spanish and English, empowering patients to manage chronic conditions effectively

Required Qualifications:

• Active Tennessee RN or LPN license
• Fully bilingual in Spanish and English (oral and written)
• 2+ years of experience in care coordination, telehealth, or chronic disease management
• Proficient in EHR systems and comfortable with telehealth technology
• Organized, compassionate, and able to thrive in a remote, HIPAA-compliant workspace
