Job Summary

A company is looking for a Care Navigator (Virtual Healthcare) to support individuals with dementia and their families through telehealth services.

Key Responsibilities

• Provide guidance, assistance, and resources to individuals and families navigating dementia care
• Conduct comprehensive assessments to understand the needs and challenges of patients and caregivers
• Collaborate with interdisciplinary teams to develop and update personalized care plans

Required Qualifications

• Associate's or Bachelor's degree required, or relevant professional experience in lieu of a formal degree
• Minimum of 2 years of professional experience working with the geriatric population
• Prior experience with dementia or cognitive impairments preferred
• Ability to work in a virtual environment with a private and quiet workspace
• Passionate and patient-centered focus supporting individuals living with dementia and their families
