Job Summary

A company is looking for a Clinical Patient Engagement Specialist.

Key Responsibilities

• Engage with prospective clients and record interactions in the CRM system, making 60-100 outbound calls daily
• Facilitate personalized care planning by conveying client preferences to the care team
• Assist clients in completing psychosocial assessments and developing actionable care plans

Required Qualifications

• Active California Licensed Vocational Nurse (LVN) license, Bachelor's of Social Work, or Medical Assistant certificate
• 3+ years of experience in healthcare, preferably with vulnerable populations
• Comfortable with technology, including video conferencing and electronic health records
• Prior experience in a remote role with a secure workspace
• Bilingual (English/Spanish) preferred
